Yes. Attach forms to specific services and configure them as required. You can also set whether the form needs to be completed every appointment or just once (e.g., a one-time medical history vs a per-visit consent).

How do clients complete forms?

Clients receive a link via the appointment notification system. They complete the form on their phone or computer before arriving. Completed forms are linked to their appointment and client profile.

What field types are available?

Nine field types: short text, long text, single choice (radio), multiple choice (checkboxes), dropdown, yes/no, signature, file upload (photos and PDFs — up to 5 files per question, 20 MB each), and info block (display-only). Fields can be grouped into named sections for logical organisation.

Can I view past form submissions for a client?

Yes. All form submissions are stored against the client profile with timestamps. You can review previous responses when a client returns. Form file uploads also appear on the client's Files and Photos tabs for your team.
